At ArchCare at San Vicente de Paúl Skilled Nursing and Rehabilitation Center, we want to do everything we can to ease the hospital discharge process, so please know that we welcome suggestions from Discharge Planners.
Although we encourage families to tour ArchCare at San Vicente de Paúl Skilled Nursing and Rehabilitation Center before they make a decision, we also understand that a visit is not always possible.
We require the following standard information prior to a bed offer:

- PRI
- Medication Record
- Level 1 Screen
- Level 2 Screen for psychiatric or MRDD referrals (only for long-term residents)

- Psychiatric Evaluation (when indicated)
- Consults
- Blood Work
- Blood & Urine Cultures
