Subacute Transitional Care

The Subacute program serves adult patients who no longer need hospitalization but still require clinical care for recovery from a complex medical condition or surgical procedure. Patients may be admitted to the Subacute Unit for:

  • Pulmonary disease, including pneumonia, chronic lung disease, or asthma
  • Cardiac conditions, including recovery from heart attack, cardiac surgery, or congestive heart failure
  • Neurological rehabilitation from a stroke
  • Fractures
  • Recovery from joint replacement surgery or soft tissue injury
  • Palliative care and pain management
  • Oncology care including chemotherapy
  • Dialysis
  • Wound management

The 28-bed unit has a dedicated healthcare team with 24-hour, on-site medical coverage and respiratory therapy, nurses with acute care experience and advanced training, and a full range of physical, occupational and speech therapy.
