What is ArchCare Advantage HMO Special Needs Plan?

This is a long-term Medicare plan designed specifically for the needs of your loved one living in a nursing home — which includes a dedicated Nurse Practitioner to coordinate and watch over their care. With our Advantage Special Needs Plan (SNP), you won’t have to deal with the complexity of your loved one’s changing care needs on your own.

Nothing is more important to us than your loved one’s health and quality of life while they live in a nursing home. That’s why, when you sign up for our Advantage Special Needs Plan, we give you a dedicated Nurse Practitioner who’ll be by your loved one’s side as their needs change and their care gets more complicated.

Beyond your dedicated Nurse Practitioner, who will be the point person for your loved one’s care, this plan covers:

  • Up to $120/month for over-the-counter medications
  • Prescription drug coverage
  • Primary care & specialist visits with no co-pay
  • Health management
  • Preventive care

With the help of your Nurse Practitioner, your loved one will get the things they need to help keep them as healthy and happy as possible in the nursing home.

Our OTC card.

As a member of ArchCare Advantage HMO Special Needs Plan, your loved one will receive a pre-paid over-the-counter (OTC) card. They can use this card to buy eligible OTC medicines and health-related items. In 2022, the OTC card benefit is $120/month (up to $1440/year).*

*Any remaining balance at the end of the month will not carry over to the next month. You can use this card at participating retailers. Please note that not all OTC items are covered. OTC items may be purchased for the ArchCare Advantage member’s use only. 

Our Advantage Special Needs Plan may be available at no additional cost to your loved one beyond their monthly premiums.

The amount they pay for ArchCare Advantage HMO Special Needs Plan varies.

  • They can receive all the benefits of SNP in one plan at no additional cost beyond their monthly Medicare plan and Part D prescription drug premiums.
  • If they received both Medicare and Medicaid, they may also be eligible for extra help paying premiums.

ArchCare Advantage HMO SNP is a Coordinated Care Plan with a Medicare contract. Enrollment in ArchCare Advantage depends on contract renewal. This plan is available to anyone with Medicare who meets the Skilled Nursing Facility (SNF) level of care. Limitations, copayments and restrictions may apply. Eligible beneficiaries may enroll in our plan at any time of year. You must continue to pay your Medicare Part B premium. Benefits, formulary, pharmacy network, premium and/ or co-payments/co-insurance may change on January 1 of each year.
