To be eligible for ArchCare Community Life, you must meet all of the following requirements:

• You must be 21 years of age or older

• You receive or are eligible for Medicaid

• You need more than 120 days of community-based long-term-care services

• You are able to live safely in the community at the time you enroll

• You reside in ArchCare Community Life’s service area, which currently includes Manhattan, The Bronx, Brooklyn, Queens, Staten Island, Westchester and Putnam County
