When enrolling in ArchCare Community Life, you will be assigned to a highly trained Care Manager. Your Care Manager will take the time to develop a personal relationship with you, your Primary Care Provider, your family and all the members of your care circle.

Your Care Manager will work with you to create a Person Centered Service Plan (PCSP) that includes input from your primary care physician or specialists. Our goal is to assist you in navigating the health care world, especially with in-patient hospital services covered directly by Medicare or Medicaid. We work together to make sure you’re getting the most from your benefits.
