Our Senior Life PACE Harlem Center is designed as a vibrant neighborhood where older adults can spend their day on their terms, supported by a fully integrated care team. From medical care and rehabilitation to social connection and daily activities, everything is coordinated under one roof to help participants remain independent, engaged, and living at home.
What is ArchCare Senior Life PACE?
Senior Life (PACE) is a long-term Medicaid/Medicare plan designed specifically for the needs of seniors like you — who need nursing home level care but want to stay at home. With Senior Life, you’ll have a dedicated Care Team to coordinate everything, so you’ll be able to live comfortably where you are, knowing you’ll get the kind of at home care and therapies you need.

- Yes, if you are 55 or older and eligible for nursing-home level care as certified by New York State
- Yes, if you require more than 120 days of community-based long-term care services
- Yes, if you are living in our service area
- And yes, if you can live safely in the community with PACE assistance at the time of enrollment

What You Get from PACE.
