What is ArchCare Senior Life PACE?
Senior Life (PACE) is a long-term Medicaid/Medicare plan designed specifically for the needs of seniors like you — who need nursing home level care but want to stay at home. With Senior Life, you’ll have a dedicated Care Team to coordinate everything, so you’ll be able to live comfortably where you are, knowing you’ll get the kind of at home care and therapies you need.

- Yes, if you are 55 or older and eligible for nursing-home level care as certified by New York State
- Yes, if you require more than 120 days of community-based long-term care services
- Yes, if you are living in our service area
- And yes, if you can live safely in the community with PACE assistance at the time of enrollment

Your care team.
Nothing is more important to us than your health and quality of life. That’s why, with Senior life (PACE), you’ll get a team to coordinate your medical needs, 24 hours a day, 7 days a week.
Your Care Team will include:
- Your primary care physician
- Your registered nurse
- Your social worker & community service worker
- Your physical, occupational, and recreational therapists
- Your dietician
- Your PACE center manager
- Your home care coordinator
- Your transportation coordinator
Besides doing regular check-ins with you through Medsign*, your dedicated Care Team will:
- Coordinate at-home physical & occupational therapy
- Work closely with all of your doctors and care givers
- Arrange your Home Health Aides
- Have prescriptions delivered to your door
- Plan at-home recreation visits
- Provide eye, foot & dental care & hearing aids
- Provide wound & palliative care
- Give nutritional advice & counselling
Visit your local PACE center.
With Senior Life, you’ll be able to visit your local Day Health Center. There, you’ll be able to get medical care at the health clinic, have meals, participate in social and recreational activities and more.

You may be eligible for Senior Life at no cost to you.
You may qualify, like most of our participants, for zero-month cost share or premium based on your income. Additionally, all ArchCare Senior Life services are provided with no co-payments or out-of-pocket expenses to you. Please call us with any questions about your eligibility for zero cost.
