The Guiding an Improved Dementia Experience (GUIDE) Model is a no-cost program that aims to improve quality of life for people with dementia, reduce strain on their caregivers, and enable people with dementia to remain in their homes and communities for as long as possible.

- Yes, if you are on Medicare as your primary insurance plan
- Yes, if you are enrolled in Medicare Parts A and B
- Yes, if you have a diagnosis of dementia confirmed by medical provider
- Yes, if you are not enrolled in Medicare Hospice benefit
- And yes, if you do not reside in a long-term nursing home

Care Coordination Services
We manage the individual’s dementia and co-occurring conditions by coordinating with healthcare providers for comprehensive care.
Interdisciplinary Care Team
A team of healthcare professionals create a personalized care plan based on the patient’s goals, strengths, and needs.
Care Navigator
We assist patients in accessing resources like home-delivered meals, transportation, home health aides, and medical care.
